Class Registration Form
• Class size is limited to 15 students and will be filled on a first come, first-served basis.
• Your class confirmation will be sent no later than May 28th. Via e-mail (if you don’t have e-mail, please include a self addressed stamped envelope and it will be mailed to you). to check class availability.
• Your registration must include a check for the class fee made out to your 1st choice teacher. Should you not get your 1st choice,
the check will need to be rewritten.
• Please indicate 1st, 2nd and 3rd choices. You must be willing to take any of the classes you list.

I understand if I am a "no show" I forfeit all monies paid. If you are confirmed in a class and cannot attend you may sell you space but please contact Class Coordinator Donna at 760.715.5518
